Dimaag — brain-mind दिमाग thought, reason. "Dimaag kharab" = grave, stigmatised label — use with care सोच, विवेक। «दिमाग खराब» = गम्भीर, कलंकित लेबल — होसियारीसाथ Man — heart-mind मन feeling, memory, desire — where distress is felt and safely spoken: "man dukhcha" भावना, सम्झना, चाहना — पीडा यहीं महसुस र सुरक्षित रूपमा भनिन्छ: «मन दुख्छ» Saato — spirit सातो can flee in fright: "saato gayo" — healers call it back (saato bolaune) तर्सिँदा भाग्न सक्छ: «सातो गयो» — धामीले फर्काउँछन् (सातो बोलाउने) Ijjat — honour इज्जत the social skin: stigma wounds it, so families hide illness सामाजिक छाला: कलंकले घाइते हुन्छ, त्यसैले परिवारले रोग लुकाउँछ

A skilled counselor speaks to the man (heart-mind) — "man ko kura" — because problems of the man are shareable, while "dimaag kharab" invites stigma. The dotted ring is saato and ijjat: fright and honour, both of which the family will be managing alongside the symptom.

The curriculum names three: dhat, possession, soul loss. A culture-bound syndrome is a pattern of distress recognised and named by a culture, coherent inside its own explanatory world.

पाठ्यक्रमले तीन वटाको नाम लिन्छ: धात, छोपुवा (आवेश), सातो जानु। संस्कृति-बद्ध सिन्ड्रोम भनेको कुनै संस्कृतिले चिनेको र नाम दिएको पीडाको ढाँचा हो — आफ्नै व्याख्या-संसारभित्र पूर्ण अर्थपूर्ण।

Healing systems your clients use, usually before you: dhami-jhankri (shamanic diagnosis and ritual), jharphuk (sweeping/blowing away affliction), vaidya and Ayurveda, lamas and monasteries, temple vows and pujas. The evidence-based stance (see the traditional-healers review below) is collaboration: healers refer what they cannot heal when they are respected, and families accept counselling more easily when it does not demand they abandon their world. The counselor's line: work alongside any practice that is not harmful; act on harmful ones (beating "spirits" out, chaining, denial of urgent medical care) — through relationship where possible, through protection systems where necessary.

The standing rule: never invent Nepali prevalence figures. For OCD, eating disorders, and sleep disorders no national figure exists — say so. Where Nepali evidence does exist (post-earthquake distress studies, the maternal-mortality suicide finding, the National Mental Health Survey 2020), cite it by name and year.
