Mental health & mental illness: the basicsमानसिक स्वास्थ्य र मानसिक रोग: आधारभूत कुरा

Before any disorder can be named, three questions have to be answered: what is mental health, where is the line between normal and abnormal, and what causes the line to be crossed. This is the CTEVT curriculum's opening block (M2 · Mental Health · Tasks 1–2).

Mental health (WHO/your class notes): a state of psychological, emotional, and social well-being in which a person realises their own potential, copes with the normal stresses of life, works productively, and contributes to their community. Mental illness: a clinically significant disturbance in cognition, emotional regulation, or behaviour that causes distress or impairment in functioning. Health is not merely the absence of illness — a person can lack a diagnosis and still be languishing.

मानसिक स्वास्थ्य (डब्ल्यूएचओ/कक्षा-नोट): मानसिक, भावनात्मक र सामाजिक तन्दुरुस्तीको त्यो अवस्था, जसमा व्यक्तिले आफ्नो क्षमता चिन्छ, जीवनका सामान्य तनाव झेल्छ, उत्पादनशील काम गर्छ र समुदायमा योगदान दिन्छ। मानसिक रोग: सोच, भावना-नियमन वा व्यवहारमा उल्लेखनीय गडबडी, जसले पीडा वा कामकाजमा बाधा ल्याउँछ। स्वास्थ्य भनेको रोग नहुनु मात्र होइन — निदान नभएर पनि मानिस ओइलाएको हुनसक्छ।

Where is the "abnormal" line? No single test decides it. Clinicians triangulate with the 4 D's: Deviance (far outside cultural norms), Distress (it hurts), Dysfunction (it breaks daily functioning — work, school, relationships, self-care), and Danger (risk to self or others). Culture sits inside every D: hearing a dead relative's voice during mourning is deviant in Berlin and expected in Bhaktapur. Frequency, intensity, and duration turn an experience into a symptom.

1 · The biopsychosocial model (Engel, 1977). Every case has three strands braided together — biological (genes, brain chemistry, physical illness, substances), psychological (coping style, thought patterns, self-concept, learning history), and social (poverty, family conflict, displacement, discrimination, support networks). The curriculum's own list — biological, psychosocial, environmental — is this model in different words.

१ · जैव-मनो-सामाजिक मोडेल (एंगेल, १९७७)। हरेक केसमा तीन धागा बाटिएका हुन्छन् — जैविक (वंशाणु, मस्तिष्क-रसायन, शारीरिक रोग, लागुपदार्थ), मानसिक (सामना गर्ने शैली, विचार-ढाँचा, आत्म-धारणा, सिकाइ), र सामाजिक (गरिबी, पारिवारिक द्वन्द्व, विस्थापन, विभेद, सहयोग-सञ्जाल)। पाठ्यक्रमकै सूची — जैविक, मनोसामाजिक, वातावरणीय — यही मोडेलका अर्का शब्द हुन्।

2 · The diathesis-stress model. A disorder appears when an underlying vulnerability (diathesis — genetic loading, early adversity, temperament) meets enough stress (loss, migration, exam pressure, disaster). High vulnerability needs little stress; low vulnerability needs a lot. This is why one sibling breaks after an earthquake and another does not — and why "why now?" is one of assessment's most powerful questions.

२ · डायाथेसिस-स्ट्रेस मोडेल। भित्री जोखिम-प्रवृत्ति (वंशाणु, बाल्यकालको प्रतिकूलता, स्वभाव) र पर्याप्त तनाव (क्षति, बसाइँ-सराइ, परीक्षा-दबाब, विपद्) भेट हुँदा विकार देखा पर्छ। धेरै जोखिम-प्रवृत्ति हुनेलाई थोरै तनाव पुग्छ; थोरै हुनेलाई धेरै चाहिन्छ। भूकम्पपछि एउटा भाइ बिरामी पर्ने, अर्को नपर्ने कारण यही हो — र मूल्याङ्कनको सबैभन्दा शक्तिशाली प्रश्न «किन अहिले?» हुनुको पनि।
